Description: 

The Federally Qualified Health Center Urban Health Network (FUHN) is an organization of ten Twin Cities Federally Qualified Health Centers (FQHCs) working in partnership with Minnesota’s Department of Human Services on Medicaid health care reform efforts.  These efforts include FUHN’s participation in a Integrated Health Partnership Accountable Care Organization demonstration project with the State of Minnesota.  Through this demonstration project FUHN participant FQHCs hope to further enhance the health care provided to their Medical Assistance patients by coupling their primary care efforts with expanded use of technology.  FUHN expects the demonstration project will improve service access, clinical quality and patient satisfaction/engagement, while reducing overall costs to Minnesota's Medicaid program. FUHN has selected OptumInsight as its business partner in this project.

The Dartmouth Institute for Health Policy and Clinical Institute published a study through theCommonwealth Fund that details FUHN's efforts as the nation's first virtual FQHC led Medicaid ACO.
