Description: 

The District of Columbia has two health homes programs. The first State Plan Amendment was approved in 2015 for a Severe Mental Illness (SMI) health home, called My DC Health Home, and the second State Plan Amendment for a chronic conditions health home, called My Health GPS was approved in 2017. 

The District of Columbia’s (District) Department of Health Care Finance (DHCF) is launched a care coordination benefit for Medicaid beneficiaries with multiple chronic conditions, called My Health GPS. As part of the District’s My Health GPS program, interdisciplinary teams embedded in the primary care setting will serve as the central point for integrating and coordinating the full array of eligible beneficiaries’ primary, acute, behavioral health, and long-term services and supports to improve health outcomes and reduce avoidable and preventable hospital admissions and ER visits. Unlike DHCF’s initial Medicaid Health Home  benefit (My DC Health Home) where individuals must have a severe mental illness to receive services, the My Health GPS program will deliver care coordination services to beneficiaries with multiple chronic conditions, enrolled in either Fee-For-Service or Managed Care. The District’s My Health GPS program began in July 2017.
