Care4Kids is a new Medicaid program that provides comprehensive and coordinated health care for children in out-of-home care in a medical home. The program is designed to meet the unique health needs of children entering out-of-home care through the child welfare system, children placed in a Court Ordered Kinship home, Level 1-5 foster home, or group home. Care4Kids provides children with primary care physicians who are trained in the needs of children in Out-of-Home Care,and also creates a team of professionals who coordinate care for the child. Care4Kids will develop a comprehensive health plan for each child that meets the child’s unique needs. 

Care4Kids began in the following counties in southeast Wisconsin on January 1, 2014:  Kenosha, Racine, Waukesha, Ozaukee, Washington, and Milwaukee Counties. The state is contracting with Children’s Hospital of Wisconsin to provide the Care4Kids program in this area of the state.
