Comprehensive Primary Care Plus (CPC+) is the largest-ever initiative to transform and improve how primary care is delivered and paid for in America. This progressive comprehensive primary care model is implemented in 18 regions and encompasses 2,851 primary care practices. This model seeks to improve access to care, quality of care, and efficiency of care. The entire model is based around Comprehensive Primary Care Functions, including Access and Continuity, Care Management, Comprehensiveness and Coordination, Patient and Caregiver Engagement, and Planned Care and Population Health. To support these changes, CPC+ includes three payment elements: care management fee, perfofmance based incentive payment, and payment under the Medicare physician fee schedule.
