The need for collaboration between primary care and public health is widely recognized, but these sectors don’t have a lot of experience working together. To address this, the de Beaumont Foundation, Duke Community and Family Medicine, and the Centers for Disease Control and Prevention have developed a new tool – A Practical Playbook: Public Health & Primary Care Together. The Practical Playbook is a free, web-based tool designed to facilitate public health and primary care integration by offering a variety of resources for primary care providers and public health officials.

In this webinar, we will explore the new tool and how it can support the implementation of community projects that aim to improve health and reduce health related costs. In addition, we hope to hear from webinar participants about cross-sectoral projects already underway and to better understand the challenges faced so we can continue to adapt the Practical Playbook to meet the needs of those in the field.

To ensure the most effective dialogue and interaction around the Practical Playbook, please consider a specific health problem in your community for which you are already working in partnership or think a partnership would be useful.
