In 2009, Legislative Bill 396 authorized the Nebraska Medical Home Pilot Program Act with the purpose of improving health care access and health outcomes for patients and containing costs in the Medicaid program. The act defined medical home for the state of Nebraska and authorized a medical home pilot program. In 2011, the Nebraska Medicaid Patient-Centered Medical Home (PCMH) pilot was launched with two participating primary care practices; the pilot ended in 2013. In 2014, a voluntary, two-year, multi-payer medical home pilot was launched with the support of the Nebraska Legislature through a Participation Agreement. Commercial insurers and Medicaid managed care plans were asked to support PCMH-recognized practices by linking payments to clinical, financial and patient satisfaction measures. The goals of the Agreement include contracting with approximately 10 clinics by the end of 2014 and 20 clinics by the end of 2015.

LB 863 - Adopt the Travel Insurance Act and the Primary Care Investment Act

This legislation establishes a Primary Care Investment Council, charged with measuring primary care spending in Nebraska recommend an appropriate level of primary care investment across public and private payers, and recommend steps to attain that target. The Council is futher instructed to issue a report to the Governor and Legislative Council on an annual basis, starting in 2023 and continuing through 2028.

Participation Agreement

This Participation Agreement is predicated on LB239 - Adopt the Nebraska All-Payer Patient-Centered Medical Home Act, proposed by Senator Wightman. The voluntary agreement provides an opportunity for insurance companies and providers to agree to support and promote the creation of PCMHs in Nebraska through the use of consistent requirements and measurements.

Nebraska Patient-Centered Medical Home Pilot

In 2009, the Nebraska Legislature, through enabling legislation initiated the Nebraska Medical Home Pilot Program Act to be designed and implemented by the Division of Medicaid and Long-Term Care (DHHS). The two-year pilot began in February, 2011, with two rural practices and 7000 Medicaid patients. The focus for the pilot was to transform the two practices into recognized patient-centered medical homes (PCMH) in order to improve health care access and health outcomes for patients and contain costs of the medical assistance program.

To reduce costs, health workers move toward more contact with patients

The days when doctors simply waited for sick people to walk into their clinics are done. Many patients with chronic diseases now find themselves in contact with clinic-employed health coaches who nudge them to take their medications, eat right, be active and assume ownership of their health. These efforts go well beyond previous patient outreach, in part because they are increasingly tied to insurance payments. The goal is to reduce costs by rewarding providers more for keeping groups of patients well rather than simply for office visits and procedures.
