Basic, evidence-based approaches to prevention that improve health outcomes and control costs such as smoking cessation and controlling high blood pressure are not uniformly practiced in primary care. What tools, interventions, or policy levers are available to states to effectively close the gap between optimal and usual care in the primary care setting in order to save lives and better manage costs? This webinar will highlight insights from the EvidenceNOW initiative, featuring representatives from cooperatives in Virginia, North Carolina, and Oklahoma who will share their experiences, challenges, and key takeaways of interest to states.
