Using Health IT To Improve Coordination for Patients With Complex Care Needs

The Health IT program is hosting a free Webinar outlining the findings from recent research that advances knowledge about how health IT can impact outcomes for patients with multiple chronic conditions seen across a variety of settings.  Included will be a review of: 

  • Coordinating care outcomes for patients with two or more chronic conditions who were discharged from a hospital to a rural primary care clinic.
  • Developing a medication management system to improve clinician practice and enhance patient engagement by identifying patients with complex medication regimens.
  • Facilitating the availability of information following care transitions including hospital and emergency department discharge and specialty care referrals into ambulatory care.
