Join the AHRQ Health Care Innovations Exchange for an interactive dialogue exploring strategies for building Health Information Exchanges (HIEs) to promote accountable care organizations (ACOs) and patient-centered medical homes (PCMHs). The exchange will begin with information about lessons learned from Delaware’s innovative Health Information Network (DHIN), an integrated, statewide health data system to support the information needs of consumers, health plans, policymakers, providers, purchasers and research to improve the quality and efficiency of health care services in Delaware. 

Learn how DHIN data is used for statewide strategic planning, for reducing unnecessary medical care and testing, and for improving care coordination. Participants will discuss key issues related to sharing information that supports ACO/PCMH implementation. 

This Web event is the third in a series designed to share novel experiences and lessons learned in putting ACO/PCMH principles into practice.

Participants will explore questions such as:

  • What are key tools and strategies to encourage providers to link to a statewide HIE? What types of participation should be required (besides opt-out options)?
  • How can specialty providers such as mental health professionals be encouraged to both provide data to and use the HIE for care coordination?
  • What strategies facilitate providing real-time data to practices?
  • What are some of the issues that need to be resolved to promote cross-state information exchange for patients who receive care outside of the state?
  • What are strategies for efficiently using HIE data for planning?
  • What types of data may be used for population-based analytics? What are some issues to consider?
