New care models and reimbursement schemes present new challenges for health care providers to manage patient populations. This presentation will share strategies for improving clinical outcomes with data, showcasing PCMH and ACO, and will feature Robert Fortini. Fortini is the Chief Clinical Officer for Bon Secours Medical Group in Richmond Virginia. He is responsible for facilitating Provider adoption of EMR, coordinating clinical transformation to a Patient Centered Medical Home care delivery model, and facilitating participation in available Pay for Performance initiatives as well as Physician advocacy and affairs. He has extensive experience in Operations and Clinical Policy development, and experience in workflow re-engineering and CQI in ambulatory care.

Objectives:   

  • Develop a strategy to measure outcomes and use analytics to improve performance
  • Recognize how to make the transition from fee-for-service reimbursement to accountable care, and how providers can automate many routine tasks including: the identification of care gaps, risk stratification to patient engagement and care management
  • Recognize how to apply technology to meet the challenges of population health management
