Facilitating PCMH RecognitionFort Lauderdale, FLNovember 7-8, 2012 About the ProgramIn response to the recommendations of stakeholders and our experience with more than 1500 NCQA Recognized primary care practices, NCQA has renovated its patient-centered medical home recognition program. The 2011 standards are more patient-centered with a significant emphasis on the planning, managing and coordinating care for patients. They have a greater emphasis on continuous quality improvement in primary care practices, and align with Federal initiatives of Meaningful Use.Join us for a thorough review of the PCMH standards. We’ll practice scoring elements and examining documentation that supports an application towards becoming a recognized patient centered medical home with NCQA.The purpose of this educational program is to help participants:Examine the essential characteristics of a recognized patient-centered medical homeIdentify the measurement and documentation criteria for each of NCQA’s updated requirementsFacilitate the NCQA survey and evaluation processSeats are limited.
