Each of the five Stakeholder Centers hosted a workshop on Tuesday, October 15 from 10AM - 12:15PM, and focused on interests and priorities of each Center's target audience. Below you will find presentation slides that were used during the workshops.

This included an expert panel representing a system-wide perspective on federal health reform programs and their implications for states and local communities. Panelists identified challenges, models, and best practices to support local health transformation through innovative clinical and financial strategies.

This session will be modeled around the best practices, challenges, and recommendations captured in PCPCC’s publication: Managing Populations, Maximizing Technology: Population Health Management in the Medical Neighborhood. Panelists discussed the key components of population health management and its impact in the medical neighborhood, including perspectives from payers, providers, and policymakers.

This session convened employers, payers, and advocates to discuss the marketplace shift toward medical home and advanced primary care employee benefit models. Panelists also engaged the audience around critical employer issues and needs to encourage collaboration, and widespread adoption of these models.

This session focused on identifying key evaluation measures and evidence that demonstrate the impact of coordinating care in the medical neighborhood. The workshop included an analysis of the latest research, including Harvard Medical School’s data identifying payment models, and quality and outcome metrics from medical home initiatives throughout the U.S.

This session featured experts in the field of patient engagement and experience, and identified key strategies that reinforce the power of patients and families in improving the health care system across the individual, practice, and policy levels.
