The PCPCC Care Delivery and Integration Center works to encourage widespread transformation of the health care delivery system and development of “medical neighborhoods” built on a strong foundation of primary care and the patient-centered medical home (PCMH). The Center identifies successful models for transformation and payment to the PCMH and helps develop training and education resources to advance these models. This session described various Accountable Care Organization (ACO) models and how the medical home has been vital to their success.

Medical Home Evaluations: Past, Present & Future
Outcomes & Evaluation Center

The PCPCC Outcomes & Evaluation Center works to demonstrate the value that primary care has on quality, costs, and waste by tracking and communicating data from PCMH initiatives, pilots, and demonstrations. The center also plays a significant role in disseminating data and results to enhance awareness for medical home success, including PCPCC’s independent publications, reports, and briefs. This session focused on the evidence-base for PCMH and evolving strategies for measurement and evaluation of the PCMH.

Health Benefit Design: Promoting the Medical Home in the Workplace

The PCPCC Employers & Purchasers Center works to engage employers, purchasers, and health plans around redesigning benefit programs that incorporate patient-centered medical home (PCMH) features and innovative payment models. This center also works to share best practices and resources based on successful models, including workplace wellness initiatives, prevention programs, and benefit structures. This session focused on the role of employers and health plans in promoting the PCMH.

The PCPCC Patients, Families & Consumers Center is dedicated to ensuring that patients, families, and caregivers are active partners in improving primary care delivery and achieving patient-centered medical home (PCMH) transformation. The center actively promotes the importance of patient communication and education; including patients as members of the care team; and sharing models of care delivery that are high-quality, compassionate, and culturally-sensitive. This session focused on how patients and families can help drive quality improvement.
