The PCPCC is excited to announce that we are now accepting early registrations for our fall 2012 conference!This conference explores in depth the process of becoming a patient centered medical home (PCMH) including redefining roles within practice staff; integrating new disciplines and services into primary care; activating patients and their families as members of the care team; coordinating care across the medical neighborhood; and utilizing technology to enhance access, share information and increase quality and safety. At this conference, we will discuss trends and issues impacting implementation of the PCMH with respect to quality improvement and patient outcomes, payment reform, and accountable care. Employers, health plans, clinicians, government and policy leaders are all working together to change our health care system through enhanced primary care. These and other stakeholders will share their experiences and strategies for collaborating to develop medical homes and improve access to medical homes for everyone in the community. Conference participants will be actively engaged in discussion with all speakers and will receive a comprehensive binder of resources including case studies and tools used by practices to enhance the care experience in the PCMH.
