PCPCC Webinar: Accountability in the Medical Neighborhood: Perspectives from Employers and Providers

Sponsored by PCPCC's Stakeholder Center for Employer & Purchaser Engagement, this webinar will explore the relationship between specialists and primary care physicians in an accountable, coordinated care environment, including medical homes and Accountable Care Organizations (ACO). Hosts will discuss key issues such as:How are these models supported by employers?What are best practices for coordinating patient referrals?What role does the patient play and how will it change their experience?Which quality metrics should be in place to enforce accountability?  Presenters include Kyu Rhee, MD, MPP, Vice President of Integrated Health Services at IBM speaking from the employer/purchaser perspective; Robert Fortini, VP, Chief Clinical Officer, Bon Secours Medical Group discussing the hospital system perspective; and Carol Henwood, DO, a family physician and avid proponent of the medical home and neighborhood integrated care model speaking from the primary care perspective. Speaker Bios: Kyu Rhee, MD, MPP serves as the Vice President of Integrated Health Services at IBM, where he has direct global responsibilities for health, safety, medical, and health benefits design and strategy services. Prior to joining IBM, Dr. Rhee was Chief Public Health Officer at the Health Resources and Services Administration (HRSA), which is the primary federal agency for improving access to health care services for people who are uninsured, isolated, or medically vulnerable. While at HRSA, he led numerous national initiatives related to prevention, quality, and public health. He also served as the Director of the Office of Innovation and Program Coordination at the National Institutes of Health (NIH), which is the primary federal agency for research. While at NIH, he led numerous initiatives related to health disparities, innovation, and partnerships. Prior to his federal government service, he worked in community healthcare settings as the Chief Medical Officer of Baltimore Medical System Inc., the largest network of Federally Qualified Health Centers in Maryland. In addition, Dr. Rhee served five years as a National Health Service Corps Scholar and Medical Director at Upper Cardozo Health Center, the largest community health center in Washington, DC. Robert Fortini, PNP is the Chief Clinical Officer for Bon Secours Medical Group in Richmond Virginia. He is responsible for facilitating Provider adoption of EMR, coordinating clinical transformation to a Patient Centered Medical Home care delivery model, and facilitating participation in available Pay for Performance initiatives as well as Physician advocacy and affairs. He has extensive experience in Operations and Clinical Policy development and experience in workflow re-engineering and CQI in ambulatory care. Before coming to BSMG, Mr. Fortini served as the Chief Medical Affairs Officer at Queens Long Island Medical Group engaged in quality and HIT adoption. He successfully applied for the first Level 3 NCQA recognized PCMH in NY State. Mr. Fortini has 30 years of experience in healthcare. He is a graduate of Fordham University and The State University of New York College of Health Professions at Upstate Medical Center in Syracuse, NY. Carol L. Henwood, DO, FACOFP is a board-certified family physician in private practice at Stowe Family Practice Clinic in Stowe, Pennsylvania, one of eight primary care sites of Pottstown Medical Specialists. She is also a staff physician at Pottstown Memorial Medical Center, where she formerly served as chair of the Family Practice Department and coordinator of the Family Practice Residency Coordinator.  Dr. Henwood leads the osteopathic profession at the national and state levels.  She is currently the Vice-President of the American College of Osteopathic Family Physicians (ACOFP), has served as Governor of the ACOFP Board of Directors, and has chaired and remains an active participant on the Board Task Force on Patient Centered Medical Home and Accountable Care Organizations.  In addition, she is a Past President of the Pennsylvania Osteopathic Family Physicians Society.  She has served on the American Osteopathic Association’s (AOA) Council on Women’s Health Issues and currently sits on the AOA Bureau of State and Government Affairs.  Most recently, Dr. Henwood participated with a delegation of osteopathic leaders who were invited to the White House briefing on the Affordable Care Act hosted by the DHHS and Senior Administration Officials.  She is frequently asked to speak on practice management, managed care, medical home and patient-centered care and health information technology.
