The 2011 standards are more patient-centered with a significant emphasis on the planning, managing and coordinating care for patients. They have a greater emphasis on continuous quality improvement in primary care practices, and align with Federal initiatives of Meaningful Use. The purpose of this educational program is to help participants:Examine the essential characteristics of a recognized patient-centered medical homeIdentify the measurement and documentation criteria for each of NCQA’s updated requirementsFacilitate the NCQA survey and evaluation processSeats are limited.
