The National Institute on Drug Abuse (NIDA) offers several tools on Screening for Tobacco, Alcohol and Other Drug Use

HealthTeamWorks developed a two-page guideline on universal substance screening for primary care settings – based on U.S. Preventive Services Task Force (USPSTF) and other best practices for routine screening for unhealthy substance use. Supplemental materials including a sample brief intervention and recommended tools for further assessment of those who screen positive on the brief screen are available.
