July 16, 2014 - Early Lessons from Advancing Care Together Program

During our July 16th  Behavioral Health SIG call, the PCPCC hosted special guest Maribel Cifuentes, Deputy Director of Colorado's Advancing Care Together (ACT) program.  Ms. Cifuentes presented on the early lessons from the four year ACT program, which was designed to test strategies to integrate primary and behavioral health care for patients with emotional and behavioral problems in diverse Colorado clinics and community mental health centers.  After creating a statewide competition, eleven innovation sites were selected and received 3-year grants of $50,000 per year to offset costs of participating in the ACT evaluation and other grant-related responsibilities. The innovation sites include urban and rural communities, community mental health centers and community health centers, public and privately owned practices, and large and small practices.   Maribel's presentation provided an overview of the program goals and implementation as well as early evaluation results from the innovation sites.  Serving as moderator, Dr. Ben Miller, opened up the call for Q&A from the larger audience.
