Primary Care Collaborative Presents Advanced Primary Care Practice Award

The Primary Care Collaborative (PCC) announced today the recipient of one of its three annual awards for 2020. The awards recognize individuals and a practice in the primary care community that exemplify excellence in providing high-value primary care and shaping the policies that support such care. The awards are being presented during the PCC’s annual conference, which is being held online this year on Nov. 30 and Dec. 1.

“The PCC is very fortunate to be associated with these primary care champions,” said Ann Greiner, PCC’s President and CEO. “We congratulate this year’s honorees and thank them for their contributions to better patient outcomes, reduced health inequities and greater value. Particularly with the challenges of a once-in-lifetime pandemic, these individuals and their organizations have shown incredible leadership, commitment and success in strengthening primary care.”

Advanced Primary Care Practice Award: Share Our Selves, an organization that provides medical and other services in Orange County, California

This award recognizes an organization that is an extraordinary primary care practice. This practice serves as a model for transformation through innovation as reflected in the Shared Principles of Primary Care, including patient- and family-centered, team-based care and high value.

Share Our Selves (SOS) has provided care and assistance to vulnerable populations in Orange County since 1970, within the four walls of the practice and way beyond in order to holistically address the needs of their patient population. With service centers throughout Orange County, it specializes in providing personalized care for the community, including medical and dental services, behavioral health, case management, financial assistance, and special holiday programs.

Its experience and innovative solutions, paired with wide-ranging services, allows the organization to address the systemic nature of poverty from many angles, upholding its clients’ basic human rights through transformational care and fighting for the dignity of all community members.
