NATIONAL HEALTH CARE ADVOCACY ORGANIZATION ANNOUNCES BRAND TRANSFORMATION, NAME CHANGE

Primary Care Collaborative aims to advance comprehensive primary care

Washington, D.C. – The Patient-Centered Primary Care Collaborative, a leading health care advocacy organization, has changed its name to the Primary Care Collaborative (PCC). The change is accompanied by a new brand identity and tagline: Convening + Uniting + Transforming.

Ann Greiner, PCC’s president and chief executive officer, announced the change earlier today at the organization’s fall conference.

The brand transformation reflects PCC’s commitment to patient-centered care and ongoing leadership in convening and uniting diverse stakeholders around policies and best practices that advance primary care.

“Since launching in 2006, the organization has earned a strong reputation for transforming health care delivery, and we felt it was time that the name and visual identity reflected this reality,” said Ann Greiner, president and chief executive officer of the Primary Care Collaborative. “As health care increasingly recognizes the value of person-centered, team-based, community-aligned primary care, this new brand embraces our role in shaping the future.”

Representing a broad group of public and private stakeholders, PCC is dedicated to advancing an effective and efficient health system built on a strong foundation of primary care and the patient-centered medical home. The organization was instrumental in the widespread adoption of the patient-centered medical home and leads the Shared Principles of Primary Care, an effort supported by nearly 350 organizations nationwide.

PCC members include physician and nursing groups, patient and consumer organizations, health plans, employers, quality organizations, and health systems.
