I hope you all had a lovely Thanksgiving break, but boy is it hard to believe 2013 is almost over! We're excited to kick off one of our new projects with the American Cancer Society that identifies the role of the medical home team in supporting patients and caregivers after receiving a cancer diagnosis, and during and after treatments.

More information about this three-part webinar series (Dec. 5th, 12th and 16th) can be found here on our project webpage, which also includes helpful checklists developed by health literacy experts to help foster dialogue between patients and their provider teams around care management and treatment plans.

Farzad Talks: Aha! Moments

After stepping down as the National Coordinator for Health Information Technology at the U.S. Department of Health and Human Services (HHS), Dr. Farzad Mostashari provided a retrospective of his many achievements and lessons learned in an "exit interview" with TEDMED.

Credited with the launch of the meaningful use of electronic health records program, Mostashari is an accomplished policy maker and data hawk that talks candidly about the many lessons he learned throughout his tenure, including how data ensures that "everyone counts."
PCPCC Cancer Care Webinar Series: Working with Care Teams After Diagnosis 
TODAY from Noon to 1:00 PM ET, the PCPCC will host: "Working With Care Teams After Diagnosis," where Dr. Amy Shaw will discuss the features of her “primary care oncology” practice, which screens for and treats lingering and late-onset effects of various cancer therapies.

Rocky Mountain High Savings for Medicaid ACO
Just in its second year, Colorado's Medicaid accountable care collaborative now covers more than half the state's Medicaid population and has yielded an additional $44 million in health care savings. An early adopter of ACO strategies, the program uses a combination of incentive payments and health IT to encourage coordinated, value-based primary care.

In addition to savings, the program has seen reductions in hospital admissions and high-cost imaging services among enrollees, encouraging other states to follow suit.

Monthly Briefing Schedule & Format Changes: There will also be no Monthly National Briefing in December! We'll return on Thursday, January 30th to reveal the PCPCC's 2014 Annual Report. 
Please note that Briefings will continue to take place on the last Thursday of the month with two important changes: 1 - registration is now required through our webinar platform; and 2 - briefings will now occur at 1 pm instead of 11 am on the last Thursday of the month.
