As we wrap up our November dedication to wellness and prevention, including health promotion and disease prevention, we're delighted to share the details of our new initiative in partnership with the American Cancer Society. 

The new patient-centered initiative focuses on supporting patients with cancer, their caregivers and families, by providing tools and education that help open the lines of communication, and identify the roles and responsibilities of the primary care and oncology teams throughout diagnosis and treatment. Can't Stop Won't Stop: Oregon's Medical Home Success
Oregon continues to lead the pack when it comes to medical home success stories, and its 450 Patient Centered Primary Care Homes (PCPCH) recently demonstrated a reduction in in emergency department visits and an increase in primary care visits.

While the improved outcomes were originally attributed to Oregon's Coordinated Care Organizations (CCO), which provide care for Oregon's 600,000 Medicaid patients, much of their success has been attributed to a solid foundation of medical home practices.

Defending Health Care: Medical Homes in the Military
As one of the country's largest purchasers of health care, the US Military Health System (MHS) spends nearly $50 billion a year on health benefits for close to 10 million beneficiaries. With 47 percent of beneficiaries reporting dissatisfaction of health services, MHS implemented a medical home model in 2009 to improve care quality and the overall patient experience.

The MHS model uses health IT tools, and has embedded nurses and physician assistants on care teams to the delivery of care and related costs. An evaluation is currently underway to assess the program's impact on cost, quality, and outcomes.

Aetna Launches Oncology Patient Centered Medical Home


In an innovative effort to help patients with cancer navigate the heatlh care system and receive quality treatment while avoiding high costs, Aetna and partners recently launched the Oncology Patient Centered Medical Home (OPCMH). 

The OPCMH uses health IT tools and evidence-based clinical practices to improve care coordination, reduce inefficiencies, and engage patients in the treatment process.
