It's a Bullish Market for Primary Care

This month is all about state innovations here at the PCPCC! This week we've seen the official launch of health insurance marketplaces to meet the needs of millions of Americans, while several states are not only working hard to meet the challenges of expanding coverage, but also improving quality and access to care through innovative collaborations and public-private partnerships. For example, the New York Times recently highlighted General Electric's medical home initiative in Cincinnati, signifying another large employer that has embraced the PCMH model.

In addition, our webinar this month will highlight Blue Cross Blue Shield Michigan's statewide PCMH program, including the history of the program, its PCMH-designation scoring process, evaluation methods, and latest results. Working with over 2,500 primary care practices, BCBSM's medical home initiative identified $155 million in savings and $26.37 lower per member per month costs over three years.

And of course, in just over a week, we'll be hosting the PCPCC's Annual Fall Conference in Bethesda, MD, where you can hear directly from the experts about state innovations, health insurance marketplace, and dozens of other topics.

New Resources Highlight State Innovations
Five States Rally for Practice Transformation: The National Academy for State Health Policy (NASHP) released a new report summarizing North Carolina's Infrastructure for Maintaining Primary Care Transformation (IMPaCT) grant. Awarded by the Agency for Healthcare Research and Quality (AHRQ), the grant provided technical assistance to support primary care transformation in four states.

Multiple Payers, Multiple Strategies: NASHP also released a new brief outlining five key strategies to support collaboration between Medicaid programs, health plans and purchasers in multi-payer medical home initiatives. The strategies include building trust between competitors, balancing flexibility and consistency, demonstrating the value's model, and using the market to drive demand.

SIM Success: The Commonwealth Fund's new brief detailed three effective strategies used by CMS State Innovation Models (SIM) grantees, including building on past reforms, using financial incentives and performance measures, and reworking health IT to deliver targeted care cost and quality data. CMS has awarded $300 million to 25 states since launching the program in July of 2012.

GE Lights Up Medical Homes in Cincinnati
The New York Times recently reported on General Electric's innovative redesign of its employee health coverage plan, which includes a substantial investment in medical homes. Over the past few years, GE has reached out to hospitals and practices in Cincinnati and has pushed for greater care coordination, increased use of electronic medical records, and improved transparency of results.

G.E.'s investment in medical homes and a pay for value approach to care delivery is already creating waves. Between 2008 and 2012, patients enrolled in medical homes saw a drop in emergency room visits by 3.5 percent and hospital admissions by 14 percent. Over the last few years, 118 practices in Cincinnati have converted to medical homes.

PCPCC Calls for Primary Care and Mental Health Integration

In a September 30 letter to Senate Finance Committee Chair Max Baucus, D-MT, and ranking member Orrin Hatch, R-UT, PCPCC’s Behavior Health Group urged Congress to support public policies that integrate behavioral and mental health care in primary care settings.
