PCPCC's Task Force on PCMH Practice Coaching

Upcoming Meetings

Dear Task Force Members,

We would like to invite you all to participate in a Task Force meeting that will be held at the Hyatt Regency Denver Tech Center in Denver, CO, on Sunday, June 8 from 2-4 PM. In addition to providing an opportunity for all of you who are doing this work across the country to meet and network, we will use this time to continue our work on the PCPCC publication on Practice Coaching and PCMH Transformation and provide updates on national and regional programs supporting primary care practice transformation (a few updates provided below).

If you are able to attend this meeting, please RSVP to Amy Gibson at agibson@pcpcc.org no later than Monday, May 5th. We encourage you all to stay and participate in the PCPCC Western Regional Conference that will be held at the same location June 9-10. Early bird registration ends tomorrow so if you are not already registered to attend, please visit

Practice Coaching and PCMH Updates
There has been a great deal of activity on the Primary Care Practice Coaching and PCMH Transformation front! We recently held a call with our Executive members to solicit input for the PCPCC's response to the CMS Request for Information on Practice Transformation in which we encouraged additional support and resources directed to practice coaching services to guide primary care practices on their journey to PCMH. PCPCC staff were pleased to participate in the recent
CMS Comprehensive Primary Care (CPC) conference held in Baltimore at which we heard about the outstanding transformation work of practices participating in that program. In addition to CMS, AHRQ continues to lead in efforts to develop the evidence-base for quality improvement in primary care as well as developing tools and resources to assist in the facilitation of primary care practice transformation. Last month AHRQ announced a funding opportunity  to "disseminate Patient-Centered Outcomes Research (PCOR) findings directly to primary care practices and to support them in implementing PCOR clinical and organizational findings."   Applicants responding to this announcement "must propose a comprehensive approach that uses evidence-based quality improvement strategies, such as practice facilitation, designed to improve the capacity of primary care practices to implement new PCOR evidence into the delivery of care." For more information about this opportunity, please visit the grants.gov website at:  http://grants.nih.gov/grants/guide/rfa-files/RFA-HS-14-008.html

Upcoming Events and Opportunities for Networking

Thanks to all of you who responded regarding your availability at upcoming PCPCC events. Although we will likely have a small group, for any of you who are attending the upcoming Medical Home Summit in Philadelphia, we will be meeting in room Commonwealth A from 7-8 a.m. on Wednesday, March 19th prior to the start of the closing plenary.  We are also planning to meet at the PCPCC Western Regional Conference in Denver, Colorado. We have tentatively scheduled to meet from 3-5 pm on Sunday, June 8, 2014, so please put a hold on your calendars. For those of you planning to attend the Western Regional Conference and would like an opportunity to provide participants with information about your practice coaching services, exhibit booths are still available. If you are interested in an exhibit booth or other sponsorship opportunities at the conference, please send me an e-mail directly and will fill you in on all the details.
