Task Force Overview



In January, the PCPCC Board of Directors officially approved the establishment of a Task Force on Practice Coaching and Technical Assistance. The Task Force has been charged with the development of a publication that will highlight experiences of organizations guiding primary care practices in their transformation to patient-centered medical homes (PCMH). The publication will discuss critical elements to practice transformation, challenges confronting practices of varying size and structure, and future direction for practice coaching in support of ongoing quality improvement and sustainability of PCMH care models. PCPCC staff are exploring various options for this publication - possibly an article for publication in the peer-reviewed literature and a more comprehensive PCPCC publication including case studies.

In addition to the publication, the Task Force will provide an opportunity for discussions on how the PCPCC can help advance efforts to transition practices to becoming an effective PCMH through addressing ongoing challenges related to care delivery obstacles, payment reform, and enhancing partnerships with patients and families in quality improvement, just to name a few. We hope that in partnership with all of you, we can set the vision for the future of practice transformation organizations and widespread PCMH implementation. Upcoming Events and Opportunities for Networking

Thanks to all of you who responded regarding your availability at upcoming PCPCC events. Although we will likely have a small group, for any of you who are attending the upcoming Medical Home Summit in Philadelphia, we will be meeting in room Commonwealth A from 7-8 a.m. on Wednesday, March 19th prior to the start of the closing plenary.  We are also planning to meet at the PCPCC Western Regional Conference in Denver, Colorado. We have tentatively scheduled to meet from 3-5 pm on Sunday, June 8, 2014, so please put a hold on your calendars. For those of you planning to attend the Western Regional Conference and would like an opportunity to provide participants with information about your practice coaching services, exhibit booths are still available. If you are interested in an exhibit booth or other sponsorship opportunities at the conference, please send me an e-mail directly and will fill you in on all the details.
