Schools IN for Summer!

Dear Members and Friends, 

With school out and Congressional recess quickly approaching, we have officially entered the dog days of summer. However, our colleagues at the Institute of Medicine (IOM) have been busy, recently publishing their Graduate Medical Education Funding report, which makes recommendations for modernizing how we train the U.S. physician workforce. We have also been busy on the workforce front, and are excited to share with you 31 new programs just added to our Patient Centered Primary Care Training Program Database. This online tool allows you to search for education and training programs throughout the country focused on team-based primary care from a wide variety of disciplines.

As reflected by the growing numbers of interprofessional training programs focused on team-based care, we are seeing a positive trend in the growth of the Patient-Centered Medical Home (PCMH) serving as a foundation for health system transformation. Providers and payers alike are seeing positive outcomes from this model of care being implemented across the country. Just last week CareFirst and Horizon Blue Cross Blue Shield of New Jersey announced millions of dollars in savings accredited to their PCMH programs.
 
In our journey towards lowered costs and improved quality and outcomes, patients and families are a critical part (and many times untapped resource) of the care team. CareFirst Medical Home Saves Millions
CareFirst's Patient-Centered Medical Home Program, launched in DC, Maryland, and Virginia in 2011, announced $267 million in savings after three years of operation. The program savings were even higher than anticipated. 

Four thousand primary care providers including primary-care physicians and nurse practitioners along with roughly one third of the 3.4 million members in the region were part of CareFirst's PCMH program. In 2013, the program experienced: 
  • 6.4% fewer hospital admissions
  • 8.1% fewer readmissions
  • 11.1% fewer hospital days
  • 11.3% fewer outpatient visits

Our Database for Patient Centered Primary Care Training Programs is growing! This month the PCPCC added 31 programs from academic and training institutions to our database originally launched last December.

PCPCC on the Road
PCPCC President, Paul Grundy, MD, MPH, will join the Family Medicine Residents and Medical Students for its 2014 National Conference on August 7th in Kansas City, Missouri. Dr. Grundy will discuss the evolution of health care, the transformation of family medicine, and the patient-centered medical home (PCMH). 

On August 8th, PCPCC CEO, Marci Nielsen, PhD, MPH will participate in a panel discussion on the patient-centered medical home and health policy at
 the American Psychological Association's 2014 Convention in Washington, D.C.

PCMH Culture Improves Care Quality
A recent study looked at claims data for more than 200,000 Horizon Blue Cross Blue Shield of New Jersey members. Results showed that practices participating in its patient-centered medical home (PCMH) program performed better than practices not included. 
 
Results revealed increased benefits for patients while deceasing costs. Additionally, 1,200 emergency room visits and 260 inpatient hospitals admissions were avoided by members involved in the PCMH. Together, these savings added up to roughly $4.5 million.
