Join a PCPCC webinar to hear Mark McClellan of the Brookings Institution discuss the important role primary care leadership plays in health care transformation, including physician-led ACOs and other payment reform initiatives. 

He will highlight major themes and recommendations from a recently released ACO Learning Network toolkit, Adopting Accountable Care: An Implementation Guide for Physician Practices. The four major themes include: 

  • identifying and managing high-risk patients; 
  • crafting referral networks; 
  • effectively using event notifications and other health IT; and 
  • engaging patients 

Dr. McClellan will also discuss ways that SGR and other payment reforms can reinforce and encourage the development of physician-led ACOs and other delivery system transformations, such as more team-based care, better use of advanced practice nurses, and the further development of medical homes.
