The Medical Home Summit is a hybrid conference, internet event and training tool; a leading forum on developing and implementing patient- and family-centered medical homes.
The patient-centered medical home (PCMH) is firmly established and widely recognized as the foundation of health care improvement and the achievement of the Triple Aim. The Sixth National Medical Home Summit, co-hosted by the Patient-Centered Primary Care Collaborative (PCPCC) will bring together the leading authorities and practitioners in the medical home field to discuss how it is working, where it is working, lessons learned, and future challenges and opportunities.
Two preconference events will highlight the work of current PCMH recognition programs, as well as lessons learned from PCMH 'early adopters,' followed by a joint plenary session with the National Population Health Colloquium. The Medical Home Summit Plenary will feature the 20-year accomplishments of the Primary Care Development Corporation (PCDC), featuring a keynote presentation from founder and CEO, Ronda Kotelchuck. Additional session topics include empowering the patient voice, an overview of current research, population health management in the medical neighborhood, employer and purchaser PCMH models, state initiatives, integration of oral health, and many more.
Three afternoon mini- summits will provide an in-depth discussion of primary care workforce policy, the evolution of the medical neighborhood, health plan support issues, and a certificate program.
