CLICK HERE TO REGISTER.Date:  January 24 - 25, 2013Time:  8:30 a.m. – 5:30 p.m. on Day One; 8:30 a.m. – 12:30 pm. on Day TwoContinental breakfast will be served both days; lunch will be served on January 23. Registration Fees: Early Bird Registration Fee: $850 on or before December 24, 2012Regular Registration Fee: $995 after December 24, 2012. More than 4,600 NCQA-Recognized Patient-Centered Medical Homes practices and over 22,000 recognized clinicians have proved the strong relevance of the 2011 PCMH Standards. The 2011 standards are more patient-centered with a significant emphasis on the planning, managing and coordinating care for patients. They have a greater emphasis on continuous quality improvement in primary care practices, and align with Federal initiatives of Meaningful Use. The purpose of this educational program is to help participants:Examine the essential characteristics of a recognized patient-centered medical homeIdentify the measurement and documentation criteria for each of NCQA’s updated requirementsFacilitate the NCQA survey and evaluation processSeats are limited.
